一、臺北市政府'衛生局委託國立陽明交通大學辦理，為瞭解本市國小學童用眼行為暨政策滿意度調查，以串聯本市學童接受專業視力檢查服務之視力檢查值，分析學童日常用眼狀況之相關性，爰辦理家長「學童用眼習慣與行為問卷暨政策滿意度問卷調查」。二、111年第1波「國小學童用眼行為調查問卷」全面採線上填寫，檢附問卷QRCode電子檔（如附件），俾利線上轉知家長於111年6月30日(星期四)前填寫線上問卷(網址：https://forms.gle/sVAre7h7wQg14hWn7)。
